The following is an itemized list of hazardous waste violations that have not been <br /> addressed for Dollar General #13592 as of May 29, 2020. <br /> Open violations from July 24, 2019 inspection <br /> Violation#106-Failed to train employees on waste handling and emergency procedures. <br /> Based on the following observations,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> 1.According to Jeffrey Delgado,Assistant Store Manager,when employees are unable to find or make a <br /> hazardous waste determination for a waste item, based on the information provided on the hand-held item <br /> scanning system, and the Dollar General Environmental Compliance poster and guidance documents posted at <br /> the hazardous waste storage area, employees will use their best judgment to make a hazardous waste <br /> determination.Jeffrey was not aware of who or the phone number to call when he has questions about a waste <br /> item's hazardous waste determination. Jeffrey was not aware of the phone number to call to access item Safety <br /> Data Sheets(SDS),which was found on the Dollar General Safety Information Center poster in the employee break <br /> room. (IMG 7653-4, 7695-6) <br /> 2. When asked how employees clean up chemical spills and manage the waste generated from those spills,Jeffrey <br /> stated spill magic is used to absorb a liquid chemical spill,which is then swept up and thrown into the trash. Jeffrey <br /> stated that if the chemical container can no longer be sold or used by the facility,the chemical container will be <br /> scanned using the hand-held scanner, placed in a bag, and then placed into the appropriate hazardous tote. <br /> According to the Dollar General Environmental Compliance Program Training guide,found in the hazardous waste <br /> storage area, "absorbent pads used during the clean up and any other clean up materials"should be placed into a <br /> re-sealable bag and then placed into the"correct[hazardous waste]tote(i.e.Acid,Alkaline, Flammable, Oxidizer or <br /> Toxic)."Jeffrey stated the used spill magic is thrown into the trash because he thought the spill magic neutralized <br /> chemicals. (IMG 7683) <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper <br /> waste handling and emergency procedures. <br /> Provide proof of re-training and training materials to the EHD for employees whose responsibilities include <br /> hazardous waste. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#108-Failed to complete,sign or date manifest; or obtain dated signature of transporter. <br /> The 2018 and 2019 uniform hazardous waste manifests are being completed using generator EPA ID number <br /> CAR000278318. However,this facility's EPA ID numbers are CAR000278317 and CAL000373076. (IMG <br /> 7678-82, 7684-93) <br /> (Note: It is recommended for the facility to have only one active EPA ID number.) <br /> The generator and waste sections of the uniform manifest shall be completed and signed and dated by the <br /> generator. The transporter shall also sign and date the manifest. <br /> Ensure that all manifests are properly completed for any transport of a hazardous waste for off-site transfer, <br /> treatment, storage or disposal. <br /> Page 1 of 2 <br />
