The following is an itemized list of hazardous waste violations that have not been addressed for Dollar General #13357 as of May 29, 2020. Open violations from August 12, 2019 inspection Violation#405-Failed to manage incompatible wastes in containers properly. 1. The following waste items were found bagged and inside a HW tote labeled as Acid HW(IMG 8171-207): -A Garnier Nutrisse Hair Color 93 Light Golden Blonde 1 kit(UPC/SKU 603084027378),which the scanner stated should be placed in the Acid HW tote. -Six Garnier Nutrisse Hair Color B4 Caramel Chocolate 1 kit(UPC/SKU 603084431045) -Two Garnier Nutrisee Hair Color 42 Deep Burgundy 1 kit(UPC/SKU 603084244140),which the scanner stated should be placed in the Acid HW tote. -A Garnier Nutrisse Hair Color 73 Dark Golden Blonde 1 kit(UPC/SKU 603084242597),which the scanner stated should be placed in the Acid HW tote. However,the hair coloring kits listed above each contained a container of colorant, a container of fruit oil concentrate, a container of rinse-out conditioner, and a container of developer, each of which is made up of a different list of ingredients.According to the package labels,the colorant contains ammonium hydroxide,which according to the DG HW guidance poster should be placed in the Alkaline HW tote. The developer contains hydrogen peroxide,which according to the DG HW guidance poster should be placed in the Oxidizer HW tote.The fruit oil concentrate and the rinse-out conditioner contain oils and fragrance,which according to the DG HW guidance poster should be placed in the Flammable HW tote. At the time of inspection, it was unclear why the hair coloring kits were classified to be placed in the Acid HW tote. Tony Yang, Store Manager, stated that DG Corporate was working on getting the kits to come up as Oxidizer HW on the hand held scanners.A document,which listed various UPC numbers for"GN Nutrisse Ultra color Unregulated Hair Color Kit,"was found in the Acid HW tote with the hair coloring kits.According to the document, each of the hair coloring kits listed was classified as Oxidizer HW, including the hair coloring kit with UPC 603084431045, which was found in the Acid HW tote at the time of inspection. -Several containers of 5 Hour Energy Extra Strength Blue Raspberry,which the scanner stated should be placed in the Acid HW tote. -Four boxes of Clairol Permanent Root Touch-Up, each containing a container of permanent color creme and a container of color activating lotion.According to the package labels, the color creme contains ammonium hydroxide,which according to the DG HW guidance poster should be placed in the Alkaline HW tote.The activating lotion contains hydrogen peroxide,which according to the DG HW guidance poster should be placed in the Oxidizer HW tote. 2.Adjacent to the HW storage shelves were five grey totes labeled as Toxic HW,two blue totes labeled as Universal Waste, and one blue tote labeled as Flammable HW, stacked on top of each other. One of these Toxic HW totes contained the following waste items(IMG 8208-8240): -A bottle of Crest 3D White Glamorous White Alcohol-Free Mouthwash,which the scanner stated should be placed in a Toxic HW tote.According to the item label ingredient list,the mouthwash contains hydrogen peroxide. According to the DG HW guidance poster, items containing peroxides should be placed in the Oxidizer HW tote. -A Scents Essential Essential Oil Blend Peppermint Sachet.According to the DG HW guidance poster, air fresheners/fragrances and items containing oils are to be placed in the Flammable HW tote. -A Trolls Disney Make Up Set,which contained two containers of nail polish.The ingredient list for the nail polish included polyvinyl alcohol, and according to the DG HW guidance poster, items containing alcohols should be placed in the Flammable HW tote. -A container of Sani 360 Mask A Doo deodorizer,which contains essential oils.According to the DG HW guidance poster, air fresheners/fragrances and items containing oils are to be placed in the Flammable HW tote. -Two 2-count packs of Refresh Mini Diffusers,which the scanner stated should be placed in the Flammable HW tote. -A 2-count pack of DG Home Scented Oil Refills,which contains essential oils and fragrance.According to the DG HW guidance poster and guidance documents, air fresheners/fragrances and items containing oils are to be placed in the Flammable HW tote. -A pack of Little Tree air fresheners,which the scanner stated should be placed in a Toxic HW tote.According to Page 6 of 7
