The following is an itemized list of hazardous waste violations that have not been <br /> addressed for TIRE & WHEEL PROS as of April14 , 2020 . <br /> Open violations from February 07 , 2019 inspection <br /> Violation #101 - Managed a hazardous waste without an identification number. <br /> This facility' s hazardous waste ID number CAL000387059 is inactive . A hazardous waste generator shall not treat, <br /> store , dispose of, transport or offer for transportation , hazardous waste without a hazardous waste ID number . Go to <br /> the DTSC website https : //www. dtsc. ca . gov/iDManifest/ReactivatelD . cfm for the form to reactivate the ID number. <br /> Call 1 -8774544012 for assistance if having trouble logging into the eVQ website to check on EPA ID number <br /> status . Immediately reactivate your State EPA ID number and submit evidence to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) . <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #119 - Failed to report program data in the California Environmental Reporting System ( CERS ) . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #605 - Failed to completely label containers or portable tanks of hazardous waste. <br /> Three 55 gallon drums of used oil and one 55 gallon drum of waste antifreeze were found on site without a legible <br /> start date . <br /> All hazardous waste containers shall be marked with the following information : <br /> - the words " Hazardous Waste" <br /> - name and address of generator <br /> - hazardous properties <br /> - physical state <br /> - composition (contents) <br /> - accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required information . <br /> Since these containers have been on site longer than 180 days , immediately contact a licensed hazardous waste <br /> hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD . <br /> Page 1 of 2 <br />
