The following is an itemized list of hazardous waste violations that have not been <br /> addressed for TIRE & WHEEL PROS as of April14, 2020. <br /> Open violations from February 07, 2019 inspection <br /> Violation #101 -Managed a hazardous waste without an identification number. <br /> This facility's hazardous waste ID number CAL000387059 is inactive. A hazardous waste generator shall not treat, <br /> store, dispose of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to <br /> the DTSC website https://www.dtsc.ca.gov/iDManifest/ReactivateID.cfm for the form to reactivate the ID number. <br /> Call 1-877-454-4012 for assistance if having trouble logging into the eVQ website to check on EPA ID number <br /> status. Immediately reactivate your State EPA ID number and submit evidence to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #119-Failed to report program data in the California Environmental Reporting System (CERS). <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #605 -Failed to completely label containers or portable tanks of hazardous waste. <br /> Three 55 gallon drums of used oil and one 55 gallon drum of waste antifreeze were found on site without a legible <br /> start date. <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste' <br /> - name and address of generator <br /> - hazardous properties <br /> - physical state <br /> -composition (contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required information. <br /> Since these containers have been on site longer than 180 days, immediately contact a licensed hazardous waste <br /> hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. <br /> Page 1 of 2 <br />
