RECEIVED BY SJCEHD (EF) on 4/14/2020
*I
DOLLAR TRF&
April 13, 2020
Sent via email to Elianna Florido at eflorido(a,sigov.org_
San Joaquin County ^
Environmental Health Dept
1868 E. Hazelton Ave
Stockton, CA 95205
Re: ADDENDUM to RTC SUBMISSION MAY 15, 2019
Dollar Tree#05403
1209 E. March Lane
Stockton, CA 95210
Dear Ms. Florido,
In response to the inspection conducted on April 9, 2019 and the amended report of April 12,
2019, for Store#05403 in Stockton, California, please see the outline below, and the exhibits
which follow, referencing existing procedures, documentation and/or actions taken:
• Violations 104 & 105: See the completed Emergency Information form received from
the Store Manager(EXHIBIT A). It was posted in several areas of the store.
• Violation 106: I have attached the most legible training sign off we could get for the
management team at 5403 (EXHIBIT B). The training guide used is the one you had
attached to your April 13, 2020, email request.
• Violations 110 & 111: Signed facility manifest#011410898FLE (EXHIBIT C).
• Violation 202: Our managers are trained and coached to put all hazardous waste spill
materials in the appropriate tote with the empty container. We do not tell our managers to
place such materials in the trash to reduce the amount of waste, nor did the District
Manager give such direction. We do train managers to use whatever amount of litter is
needed based on the amount of spill.
We are adding a requirement to the hazardous waste promo
The manager with primary responsibility for the accumulation area, must follow-up on
any empty containers to verify where/how the contents were used or spilled. If a spill
cleanup, there should be cleanup materials bagged with the empty container. If not, the
manager must determine who placed the empty container in the tote and verify
where/how the contents were used or disposed of.
Any exceptions must be corrected and noted on the weekly inspection form.
CORPORATE HEADQUARTERS
500 Volvo Parkway Chesapeake, Virginia 23320 Tel 757-321-5000 Fax 757-321-5292 www.dollartree.com
