The main concern is the waste in the <br /> second drum before it reaches the third drum. I have concerns with heavy metals and fish toxicity. Please <br /> provide something to address these concerns. <br /> How was the bucket of oil water handled? <br /> Violation 106, Failed to train employees. Provide proof of training to the EHD for employees. <br /> Violation 113, Failed to keep copies of consolidated manifesting receipts for three years. The maintenance <br /> contractor violation is still open. The facility needs to follow the requirements listed in the violation for their <br /> own waste hauled off site. The EHD has created a bill of lading form that is acceptable. This was sent to you <br /> earlier. The form is not required but if the facility uses it, violations 113 and 608 will also be closed out. <br /> Violation 608-Failed to properly manage used oil and fuel filters.
