Date: <br />Manufacturer training: <br />OVERALL INSPECTION NOTES AND COMMENTS <br />NOTES <br />On site for tank removal of a 10,0000 gallon diesel tank. <br />Witnessed tank removal, piping and sampling tank pit and stockpiled soil, no noticeable rust was observed outside the tank <br />and holes were not observed at the bottom on the tank when it was removed. <br />Verified no signs of leakage, stains or contamination. <br />The only piping observed was running horizontally to the tank approximately 6 feet. <br />- Tank pit measurements 31ft (length) x 13ft(width) x 10ft(depth) <br />- Soil Stock pile volume approximately 50 cubic yards <br />LEL: 01 <br />Oxygen: 20.89% <br />Tank was cleaned and triple rinse <br />Dry ice was used to inert the tank. <br />Sampling was conducted by Lucy Mulvey (Geologist) of AECOM Sacramento. <br />Two stock piles samples were taken labeled SP -01 and SP -02. <br />Two samples was taken below the tank pit 2 feet into the native soil, one at each end of tank labeled TP01 and TP02 <br />One sample was taken at the middle of the tank pit 2 feet into the native soil labeled TP03.One sample was taken at the tank <br />pipping, 2 feet into the native soil labeled Dispenser. <br />Discussed sampling and analytical requirements with AECOM <br />Verified that the city permit was obtained prior to removal. <br />No noticeable discoloration was observed. <br />The tank was transported by Adams Services Inc. <br />The tank pit was backfill with 3/8 pea gravel self compact to 90%. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />of having them sign. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
