On March 28, 1994, 1 met with <br /> you to discuss the discrepancies noted in the file that prevent PHS/EHD from issuing a closure certification on <br /> the site. <br /> On April 6, 1994, the PHS/EHD Technical Review Committee (TRC) reviewed the tank removal and subsequent <br /> laboratory results as per your request of March 28, 1994. The TRC concluded that additional investigation is <br /> ex <br /> necessary in order to determine the tent of the contamination noted at the site. <br /> The following is a chronology of the file_ <br /> 11/7/86 The San Joaquin Local Health District (now known as San Joaquin County Public <br /> Health Services, Environmental Health Division) performed a site evaluation pursuant <br /> to the California Underground Storage Tank Law [California Health and Safety Code <br /> Section 25283, 25291, 25292]. <br /> The tenant. Ray Bianchi, stated that the underground storage tank would be closed in <br /> place. Mr. Bianchi was notified that closure in place would require concurrence from <br /> the Stock-ton City Fire Department. <br /> 05/18/87 The Health Department received a telephone call requesting information on the <br /> procedure for closure (removal) of an underground storage tank. <br /> 48/12/87 The Health Department received a closure plan for tank removal. <br /> 08/26/87 The Health Department reviewed and approved the closure plan. The contractor, <br /> Western deter, was notified that the closure plan was ready; left message with <br /> secretary who stated "Bill" would be in to pick up the plan. <br /> 10/15/87 The Health Department was on site for the tank removal this date. The tank was set in <br /> a concrete vault with the northern side of the vault having a dirt wall. No evidence of <br /> holes in tank were noted; sample was taken 6 feet into the north bank at the bottom of <br /> the vault. <br /> 12/01/87 The Health Department received the laboratory results taken from tank removal. Soil <br /> sample results indicate contamination of 5 ppm Benzene and 8 ppm Xylene. Other <br /> petroleum hydrocarbons was also noted as Total Petroleum Hydrocarbons, Gasoline <br /> A ni.i%ion of San Joaquin County Health Care Services <br />
