This application for a National <br /> Pollutant Discharge Elimination System(NPDES) permit describes the proposed discharge of excess <br /> treated groundwater to the West Side Irrigation District's Upper Main Canal. Groundwater is proposed <br /> to be treated and discharged to a stormwater percolation basin which also receives stormwater and <br /> production well effluent. The combined discharges to the stormwater basin in excess of the infiltration <br /> capacity of the stormwater basin are proposed to be discharged to the irrigation canal. <br /> Based on staff review of the received forms, this application is incomplete. The following information is <br /> needed in order to consider your application complete: <br /> • Because the facility will discharge to surface water under an NPDES permit, you are required to <br /> submit completed EPA General Form 1 and EPA Form 2D, as well as the State Form 200. Your <br /> application package did not include Form 1 and the accompanying figures. <br /> • Both the state and federal forms are required to be signed by a general partner or the proprietor of <br /> the partnership. Section VII of Form 200 requires a signature. <br /> Copies of submitted Forms 200 and 2D are enclosed. Flagged pages show where the forms are <br /> incomplete.
