DOCUMENTATION <br /> The implementation of the SSHP must be documented to assure employee participation <br /> and protection. In addition, the regulatory requirements must be met for recordkeeping on <br /> training, medical surveillance, injuries and illnesses, exposure monitoring, health risk <br /> information, and respirator fit-tests. Documentation of each employee's activities is maintained <br /> by the HSO in Pleasant Hill, California. <br /> Documentation of the implementation of this plan will be accomplished using SSHP <br /> Attachments A through E. Attachment A must be completed by each BC employee at the <br /> initiation of field work for the project. The site safety officer is responsible for ensuring that <br /> each BC employee has completed this form, and for submitting copies to the HSO. The site <br /> safety officer is also responsible for completing the other attachments as required for a specific <br /> project.
