The following is an itemized list of hazardous waste violations that have not been <br /> addressed for Dollar General #14661 as of May 29, 2020. <br /> Open violations from August 14, 2019 inspection <br /> Violation#103-Failed to retain hazardous waste determination including waste analysis on site for 3 <br /> years. <br /> When asked how employees determine which of the differently categorized hazardous waste(HW)totes a waste <br /> item belongs in, David Barajas, Store Manager, said each item is scanned using a hand-held scanner,which will <br /> sometimes state which HW tote the item belongs in. David indicated that when the scanner does not specify which <br /> HW tote an item belongs in, employees will read the item's label and ingredients, as well as the Dollar General <br /> (DG) HW guidance poster and documents,which provide product examples and key words for each of the HW <br /> categories,to determine which HW tote an item belongs in (see photos IMG 8283, 8293).When asked what <br /> employees do if they are still unsure about which HW tote a waste item belongs in, David stated he will call contact <br /> his District Manager, Steve Bohnak, or Eric Voyles, DG Director of Facilities Management. David also mentioned <br /> that he is able to submit a ticket notification using the facility's computer to notify DG Corporate if the hand-held <br /> scanner does not provide a HW classification for an item when the item is scanned. <br /> The waste items listed below were observed in the HW totes, but at the time of inspection it could not be <br /> determined how the HW determinations for each of these items were made.The hand-held scanner did not specify <br /> which HW tote the items should go in, and the DG Environmental Compliance HW poster and guidance <br /> documents did not list these types of items: <br /> 1. The following waste items were found in a tote labeled as Toxic HW(IMG 8287-8, 8307-9, 8311): <br /> -A DG Home Cherry Blossom Fragranced Candle <br /> -A broken wax candle of an unknown brand <br /> The business operator does not have hazardous waste determination records that support the current handling <br /> and disposal of the waste. <br /> A business operator shall keep records of any hazardous waste determinations made in accordance with Title 22 <br /> California Code of Regulations(CCR)section 66262.11 for at least three years from the date that the waste was <br /> last sent to an on-site or off-site treatment, storage, or disposal facility. The records may include Safety Data <br /> Sheets(SDS),waste test results or other hazardous waste determination documentation. <br /> Assemble the hazardous waste determination documentation for the above listed waste observed at the time of <br /> inspection, maintain on site and submit a copy to the San Joaquin County Environmental Health Department <br /> (EHD). <br /> If hazardous waste determination documentation is not available, immediately make a hazardous waste <br /> determination and begin managing the above listed wastes as specified in Title 22 CCR. Use Safety Data Sheets <br /> (SDS),waste sampling and test results or other knowledge to support your hazardous waste determination. Waste <br /> testing must be done using methods specified in Title 22 CCR including sections 66261.20-24. Submit a <br /> statement and supporting documentation with your hazardous waste determination. <br /> Demonstrate current storage and labeling for wastes determined to be hazardous wastes. Provide disposal <br /> records for wastes determined to be hazardous wastes and stored longer than the number of days specified in Title <br /> 22 CCR for your hazardous waste generator status. <br /> Page 1 of 3 <br />
