The facility did not have bills of lading for the shipment of the used/damaged electronics. <br /> According to a 2/8/2005 letter from the Department of Toxic Substances Control (DTSC)to Michael Dorsey of <br /> the California CUPA Forum, DTSC retains the primary responsibility for implementing the hazardous waste <br /> program for retailers participating in a "takeback" program: (accepting universal wastes when selling products). <br /> -The facility is shipping used lithium ion batteries, returned to the store by customers, via UPS to Battery <br /> Sol utions/Cal12Recycle, 618 E Auto Center Dr. Rm 111, Mesa, AZ 85204. A bill of lading from 8/19/2019 for the <br /> shipment of 19 pounds of lithium ion batteries was found at the time of inspection. According to a 2/8/2005 letter <br /> from the Department of Toxic Substances Control (DTSC)to Michael Dorsey of the California CUPA Forum, <br /> DTSC retains the primary responsibility for implementing the hazardous waste program for retailers participating <br /> in a "takeback" program: (accepting universal wastes when selling products). <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended on <br /> 5/29/2020 and replaces the initial report. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 6/28/2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
