An apology is offered for any inconvenience caused by this <br /> omission. The Revised Conditions of Approval are attached to this <br /> letter for your review. <br /> All other Conditions of Approval remain unchanged from the original <br /> approval letter dated october 8, 1992. Because this action <br /> involved additional Conditions, there will be a new ten day appeal <br /> period.
