The following is an itemized list of hazardous materials violations that have not been <br /> addressed for QUEST INDUSTRIES LLC as of August 10, 2020. <br /> Open violations from June 01, 2020 inspection <br /> Violation#2- Failed to complete and electronically submit a hazardous materials business plan. <br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System <br /> (CERS)for 2020. As of January 1, 2013, all businesses were required to submit all business plans electronically. <br /> Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to <br /> the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#5-Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> The hazardous materials inventory has not been submitted for 2020 and three 55 gallon drums of acetone and <br /> hydrochloric acid waste have not been reported (actual volume of each waste was not available/the contents of <br /> each drum was not available). Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#6-Failed to complete and/or submit a site map with all required content. <br /> The maps were not submitted for 2020 and do not show/address: <br /> 1. Access and exit points (facility and structures). <br /> 2. Emergency response equipment (spill control). <br /> 3. Hazardous material handling and storage areas waste acetone, waste hydrochloric acid, kerosene, waste paint, <br /> propylene glycol, glass coating powder waste, limonene degreaser, polyethylene coating, . <br /> 4. Multiple site maps are not numbered. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as "NA".
