The following is an itemized list of underground storage tank violations that have not <br /> been addressed for WESTERN FOOD & FUEL as of April 15, 2020. <br /> Open violations from December 03, 2019 inspection <br /> Violation#209 -Failure to conduct the designated UST operator visual inspection at least once every 30 <br /> days. <br /> OBSERVATION The designated operator failed to document all the alarms from the attached alarm history on the <br /> [DATE] designated operator inspection report and failed to check that they were responded to appropriately. The <br /> missing alarms include: a fuel alarm and sensor out alarm on all sensors on 6/3/19. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the inspection report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> OBSERVATION The designated operator inspections for the following months were performed more than 30 days <br /> after the previous inspection: <br /> - Initial inspection on 12/24/18, next inspection performed on 1/25/19 (32 days) <br /> - Initial inspection on 1/25/19, next inspection performed on 2/25/19 (31 days) <br /> - Initial inspection on 8/9/19, next inspection performed on 9/9/19 (31 days) <br /> REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br /> the previous inspection. <br /> CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br /> frames. <br /> OBSERVATION The designated operator failed to check and note all required items on the visual inspection <br /> reports. The incorrect and missing information included the following from the inspection reports: <br /> -Section VI (owner/operator signature, name, and date signed)was left blank <br /> -The questions regarding the fill cap and fill pipe were left blank for the diesel tank <br /> -The diesel tank was not included in the three spill container questions <br /> -The reports document that sump sensors were properly located, but do not list any sump sensors inspected <br /> -The alarm history on a report is dated 11/6/19, but the report had a date of 5/24/19. A separate 5/24/19 inspection <br /> report was found on site with a matching dated alarm history. <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 3 of 4 <br />
