<br /> This document outlines the procedures to be followed when profiling non-hazardous waste for disposal and/or treatment at Forward.
<br /> Waste acceptance is a four step procedure:(1)Establish an account at Forward;(2)Determine and perform waste profiling require-
<br /> ments:(3)Complete,sign,and submit a Waste Profiling Form along with laboratory analysis and chain of custody documentation for
<br /> Forward to review;(4)Upon Forward's approval of waste,sign and submit a Payment Terms Contract_
<br /> ESTABLISH AN ACCOUNT
<br /> An account should be established with Forward prior to submitting the waste acceptance package- Customers may either(1)establish
<br /> credit with Forward or(2)set up a prepay account. A Notification of Credit Status explaining credit limits will be sent to the applicant
<br /> following credit review. Prepay accounts are paid by cash,money order,certified check,or approved company check_Payments can
<br /> be made prior to waste deliveries at our office or at the landfill upon delivery.
<br /> DETERMINE WASTE PROFILING REQUIREMENTS
<br /> The waste generator is responsible for determining waste classification based on site knowledge,regulatory requirements,and analyti-
<br /> cal data. All analysis must be performed on representative samples. Forward requires the following analytical profiling at a mini-
<br /> mum for each type of contamination and/or waste listed below. Please contact one of our customer service staff for profiling require-
<br /> mcnts of wastes not listed below.

<br /> In additional to the above requirements,the following analysis may be required:
<br /> - The 96 Hour Acute Aquatic LC50(fish bioassay)is required for contaminated soil with c)ncentrations of:
<br /> (1)TPH as gasoline,-i,5,900 ppm;(2)TPH as diesel>20,000 ppm;(3)TPH as motor oil > 10,000 ppm.
<br /> - The Wet Extraction Test(WEI)is required for a metal with a total level 10 times greater than the SCLC.
<br /> Please refer to the Acceptable Limits section below for a list of Forward's STLC and TT X levels for each metal. These levels
<br /> have been mandated by Title 22 of the California Code of Regulations. Note that ten tim!s the chromium STLC would be
<br /> greater than the TTLC. As a rule,if the TTLC for chromium is over 100 mg/kg,the STL^will be required.
<br /> SUBMIT A WASTE PROFILE FORM/PAYMENT TERMS CONTRACT
<br /> A signed Waste Profile Form must be submitted with all certified laboratory analysis and chair of custody documentation. Forward's
<br /> staff will promptly review the submitted materials and notify the customer upon approval. Plc,se refer to instructions on completing
