A Notification of Credit Status explaining credit limits will be sent to the applicant <br /> following credit review. Prepay accounts are paid by cash,money order,certified check,or approved company check_Payments can <br /> be made prior to waste deliveries at our office or at the landfill upon delivery. <br /> DETERMINE WASTE PROFILING REQUIREMENTS <br /> The waste generator is responsible for determining waste classification based on site knowledge,regulatory requirements,and analyti- <br /> cal data. All analysis must be performed on representative samples. Forward requires the following analytical profiling at a mini- <br /> mum for each type of contamination and/or waste listed below. These levels <br /> have been mandated by Title 22 of the California Code of Regulations. Note that ten tim!s the chromium STLC would be <br /> greater than the TTLC. As a rule,if the TTLC for chromium is over 100 mg/kg,the STL^will be required. <br /> SUBMIT A WASTE PROFILE FORM/PAYMENT TERMS CONTRACT <br /> A signed Waste Profile Form must be submitted with all certified laboratory analysis and chair of custody documentation.
