The completed VCA was sent to <br /> Mr. McDougall under a DTSC cover letter dated June 16, 2004 (cover letter enclosed) <br /> that outlined requirement of an advanced payment of$7391.00. On July 27, 2004 <br /> DTSC issued a request letter (enclosed) to Mr. McDougall that identified DTSC's efforts <br /> in overseeing necessary investigation, cleanup, and completion of a Preliminary <br /> Endangerment Assessment equivalent report. DTSC identified in this letter the absence <br /> of a record showing advance payment of the cost estimate and required payment in full <br /> within 7 calendar days of receipt of the request letter. As of August 20, 2004 DTSC has <br /> no record of submittal of this payment. Therefore, DTSC is referring this matter to your <br /> offices for follow-up and any necessary enforcement action. <br /> (9 Printed on Recycled Paper <br />
