Facility kept HW on site for greater than 90 days. 1 - 55 gallon drum storing used antifreeze had an <br />accumulation start date of 4/11/10. Immediately, dispose of the antifreeze according to title 22 <br />hazardous waste regulation and submit disposal record to this office within 30 days of receiving this <br />report. <br />31. Facility lack a tank assessment for the used oil storage tank. Pursuant to CCR section 66265.191 <br />a facility must have a written tank assessment reviewed and certified by a independent, qualified <br />professional engineer registered in California. Immediately prepare a tank assessment document and <br />have it certified by a PE and submit to this office for review within 30 days of receiving this report. <br />45. Facility's Contingency plan is incomplete. The emergency phone numbers in the contingency plan <br />is not current. Immediately, update all the information in your contingency plan and keep it current at all <br />times. Provide proof of corrective action within 30 days of receiving this report. Pursuant to CCR section 66265.16 <br />personnel training record must at least include the following information, name of employee, job title, job <br />description, and the training received.
