SUMMARY OF VIOLATIONS <br />_(CLASS I, CLASS 11, or MINOR - Notice to Compry}1 <br />Item # <br />Remarks <br />605 <br />CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br />One black plastic 1 gallon bucket containing used oil was observed missing the words "hazardous waste," the name <br />and address of the generator, the physical state, the hazardous waste properties, the accumulation start date and <br />the contents. All hazardous waste containers shall be marked with the following information: <br />- the words "Hazardous Waste" <br />- name and address of generator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br />information. <br />This is a repeat violation, Class II. <br />608 <br />CCR 66266.130 Failed to properly manage used oil and fuel filters. <br />One black steel 55 gallon drum containing crushed used oil filters was observed containing oil and not closed. Used <br />oil filters shall be drained of free-flowing oil, stored in a closed, rainproof container, labeled as "drained used oil <br />filters," and marked with an accumulation start date. Immediately drain the drum of the oil and close the drum. <br />Ensure that all used oil filters are managed in this way. Submit a statement to the EHD once this has been <br />corrected. <br />This is a Class II violation. <br />3050 <br />On 11/13/2017, on manifest 3142288, Forward Landfill received 9 tons of lime waste with a pH of 12.8. <br />On multiple occasions between April 2014 -December 2015, Forward Landfill accepted waste from Woodland <br />Biomass containing hazardous waste levels of copper. Per notice of violation from the Central Valley Regional <br />Water Quality Control Board dated 6/20/2016, Forward Landfill received hazardous waste during the following <br />quarters: <br />2nd Quarter 2014: 48mg/l, 3rd Quarter 2014: 43 mg/I, 2nd Quarter 2015: 40mg/l, 2nd Quarter 2015: 31 mg/l, 3rd <br />Quarter 2015: 82 mg/I, 4th Quarter 2015: 31 mg/I, 4th Quarter 2015: 32 mg/l. <br />A facility must be permitted by the department in order to treat, store or dispose of hazardous waste; California HSC <br />25189.5 (d).
