A copy of his Hazwoper <br />Certificate is attached. <br />#60, #61 & #62 Manifest 002099347JJK — A signed copy was on had during inspection signed <br />of by TSDF but it was a copy of the Designated Facility to Generator Copy. A copy of the <br />Designated Facility Copy was requested and a copy was sent to our site. <br />Manifest 00077757OGBF — A copy of the Designated Facility Copy was sent to TSDF. Copies <br />are attached. <br />The following manifests were noted on the summary violation report that we did not have <br />them on file. Our records did not show that any waste was generated from Forward with these <br />manifest numbers in 2009. Further investigation found the following: Manifest # 003208361JJK, <br />000320835911K were found to not be waste Generated from Forward Landfill. These manifests <br />were generated by THREE WAY CHEVROLET. DTSC was notified — a letter was sent to DTSC <br />regarding this mistake for correction. Forward Landfill only generated 3 manifests in 2009. <br />DTSC will correct this in their files. <br />ABOVEGROUND PETROLEUM STORAGE INSPECTION REPORT <br /># 16 SPCC plan was signed in the presence of inspector the same day of 6/10/10. <br />#53 The inspector found the information in the current SPCC plan on 6/10/10. <br />If you have any further questions or concerns please contact me at 209-982-4298.
