Facility lack a active EPA ID number. The one that is in use now (CAL000190080) is inactive. Immediately fill out a Department of Toxic Substance Control form 1358 and activate the EPA ID number. Submit proof of corrective action to this office by 7/10/10. #31. Facility lack a tank assessment for the used oil storage tank. Pursuant to CCR section 66265.191 a facility must have a written tank assessment reviewed and certified by a independent, qualified professional engineer registered in California. Immediately prepare a tank assessment document and have it certified by a PE and submit to this office for review by 7/7/10. #49. Facility's training record is incomplete. Pursuant to CCR section 66265.16 a facility must keep at least the following information as training record, name of employee, job title, job description, and training received. Provide proof of corrective action to this office by 7/7/10. #60. Facility could not locate the signed copy for the following manifest 000777570GBF, 002099347JJK, 003208359JJK, and 003208361JJK. Pursuant to California Code of Regulation section 66262.40(a) a facility must keep signed copy on site for 3 years. Immediately locate the above mentioned manifest and submit a copy to this office by 7/11/10. #61 and 62. Facility failed to determine the status of the HW pursuant to CCR section 66262.42(a) when the signed copy of the above manifest was not received in 30 days, and also pursuant to CCR section 66262.42(b) did not file an exception report to the department. Immediately make an effort to determine the status os the HW and if unsuccessful file an exception report to the department.
