Forward Landfill <br /> San Joaquin County -3- 16 May 2017 <br /> Leachate, LCRS Sump, Pan Lysimeter Monitoring: <br /> Effective I July 2017, Republic Services Inc. shall implement semi-annual monitoring for pH <br /> and dissolved inorganics/metals (as identified in Table VIII of MRP R5-2014-0006) from all <br /> leachate sumps and pan lysimeters associated with Future Units 4,6,8,10,13 and 14. These <br /> leachate sumps are identified as WMU FU-04, 06, 08, 13, and 14. The pan lysimeters are <br /> identified as FU04, FU05, FU06 and FUIO. The results of the monitoring shall be included with <br /> the Quarterly Monitoring Report corresponding to the quarter in which the monitoring occurred. <br /> Groundwater Sampling: <br /> Effective I July 2017 and continuing quarterly, Republic Services shall analyze groundwater <br /> samples collected from existing upgradient groundwater monitoring well AMW-2 and existing <br /> downgradient monitoring wells AMW-1, AMW-4 and AMW-7 for the dissolved inorganics/metals <br /> listed in Table VIII of Monitoring and Reporting Program R5-2014-0006. The results of the <br /> monitoring shall be included with the Quarterly Monitoring Report corresponding to the quarter <br /> in which the monitoring occurred. <br /> Effective Immediately, Republic Services shall report the tonnage and sampling results for all <br /> ash received at the landfill on a quarterly basis. The tonnage of ash and sampling results shall <br /> be tabulated and include the regulatory threshold limits for all waste constituents such that <br /> determination of the waste as non-hazardous waste is readily identifiable. The tabulated results <br /> shall be submitted with the Quarterly Monitoring Reports. <br /> Any technical report required herein that involves planning, investigation, evaluation, <br /> engineering design, or other work requiting interpretation and proper application of engineering <br /> or geologic sciences shall be prepared by or under the direction of persons registered to <br /> practice in California pursuant to California Business and Professions Code sections 6735, <br /> 7835, and 7885.1. As required by these laws, completed technical reports must bear the <br /> signature(s) and seal(s)of the registered professional(s) in a manner such that all work can be <br /> clearly attributed to the professional responsible for the work. <br /> Water Code section 13268(a)(1) provides that any person failing or refusing to furnish technical <br /> or monitoring report information as required by Water Code section 13267(b), or falsifying any <br /> information provided therein, is guilty of a misdemeanor and may be liable civilly for an <br /> administered imposed liability of up to$1,000 per day for each day in which the violation occurs. <br /> Failure to submit the technical report, or submittal of an incomplete report, may subject Republic <br /> Services to further enforcement action by the Central Valley Water Board, including the <br /> imposition of administrative civil liability or referral of the matter to the District Attorney. <br /> All reports shall be uploaded to GeoTracker and an upload certification shall be e-mailed to <br /> centralvalleysacramento@waterboards.ca.gov. To ensure that each submitted report is routed <br /> to the appropriate staff, please include the following information in the body of the email: Attn: <br /> Brendan Kenny, Compliance Unit, the Date and Title of the report, and the facility's CIWQS <br /> place ID (CW-225098). <br />
