<br /> Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three
<br /> years and have them readily available for review. If the generator did not receive a copy of the manifest with the
<br /> handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35
<br /> days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or
<br /> the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive
<br /> a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial
<br /> transporter,the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and
<br /> a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those
<br /> efforts. Exception reports shall be submitted to:

<br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. Submit to
<br /> the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC.
<br /> This is a repeat violation, Class II.
<br /> 118 CCR 66265.16(d) Failed to maintain complete personnel training records.
<br /> Employee training records for the management of hazardous waste were incomplete. Records that document that
<br /> training had been given to facility personnel was not available for reivew. In addition,the manager, Mark Hoyle,
<br /> indicated that he had not yet received the annual refersher required for employees.
<br /> Employee training records shall include:
<br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each
<br /> employee filling each job;
<br /> 2. written job description for each position listed above,-
<br /> 3. written description of the type and amount of both introductory and continuing training that will be given to
<br /> each person filing positions listed above-,
<br /> 4. records that document that the training has been given to and completed by facility personnel.
<br /> Training records on current employees shall be kept until closure of the facility. Training records on former
<br /> employees shall be kept for at least three years from the date the employee last worked at the facility. Ensure that
<br /> employee training is properly documented. Submit proof to the EHD that everyone(including Mark Hoyle) has been
<br /> trainined with the annual refresher.
<br /> This is a repeat violation, Class Il.
