REPEAT VIOLATION <br />135. The complete Return to Compliance paperwork for the June 29, 2011 inspection was not <br />submitted. Complete training records were not submitted- The operator of the site or facility which <br />receives an inspection report shall submit a written response to the department or the local officer or <br />agency within 60 days of receipt of the inspection report, or with a shorter time as the department or <br />local agency requires This response shall include a statement documenting the corrective actions that <br />have been or will be taken by the operator. Immediately submit all necessary follow up paperwork for <br />the 2011 hazardous waste generator inspection to the EHD. <br />601. Two 5 gallon buckets of metal fines were observed in the shop with accumulation start dates of <br />March 5, 2012. Facilities who generate 1000 kg or more of hazardous waste per month and or exceeNl� <br />N 1*0j <br />6000 kg of waste stored on site at any time may not store waste on site for longer than 90 days. <br />Immediatelycontact a licensed hazardous waste hauler to dispose of this waste under manifest and <br />submit a copy of the manifest to the EHD by August 19, 2012, REPEAT VIOLATION <br />Notice to Comply - Minor violations' (132, 133, 610) <br />132. Daily inspections are not being conducted on the used antifreeze tank. The facility is only <br />conducting weekly inspections.
