SUMMARY OF VIOLATIONS CLASS I, CLASS 11, or MINOR -Notice to Comply) ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION Class 11 violations: (117, 122, 123, 124, 135, 601) 117. The training records on site did not include a description of the training to be given to each job position. Training records shall be maintained on site until closure of the facility and shall include: - name & job title of each employee - description for each job title listed, including requisite skill, education, qualifications, and duties - description of training to be given to each position (both initial & annual refresher) - records that training was completed for each employee - description of the training program Immediately update the training records to include all of the required information and submit a copy of the updated training records to the EHD by August 19, 2012. REPEAT VIOLATION 122, 123, & 124. Manifest number 002740959SKS (3/6/12) was found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
