The contingency plan did not have an emergency coordinator or <br />alternate emergency coordinator. The contingency plan must include: <br />1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or non -sudden <br />release of hazardous waste to air, soil or surface water at the facility; <br />2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and <br />State and local emergency response teams to coordinate emergency services; <br />3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency <br />coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and all <br />others in order in which they will assume responsibilities; <br />4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities; <br />5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br />6. Current telephone number for State Office of Emergency Services. <br />If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br />plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br />are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to fulfill <br />the requirements of this chapter. Submit a copy of the contingency plan to the EHD. This was corrected on site. <br />This is a repeat violation, Class ll. <br />114 <br />CCR 6626223(a)(4) Failed to send generator manifest copies to DTSC within 30 days. <br />Generator copies of manifest numbers: <br />-004211644SKS <br />-004423157SKS <br />were not sent to DTSC within 30 days of shipment of hazardous waste. The generator manifest copy shall be <br />submitted to DTSC for every shipment on a manifest when California is either the generator state or the destination or <br />consignment state. The generator manifest copy shall be mailed to: <br />DTSC Generator Manifests <br />P.O.
