(209) 944-21.16 Dennis Fields <br /> Cause of the spill has been determined to be a malfunction of a flow switch <br /> as well as operator error. The contacts in the flow switch, which activate <br /> the pump that empties the retort, were not making contact 100% of the time. <br /> This prevented complete emptying. The failure of the engineer to utilize <br /> the drip pump, a secondary procedure to verify complete emptying, was the <br /> other cause. Use of both pumps would ensure complete emptying, as it is <br /> unlikely that both flaw switches would fail simultaneously. <br /> Measures for correction have been completed. The flaw switch was repaired <br /> and the treating engineers have been instructed to use the drip pump after <br /> every treating cycle. A sight glass on the retort will also be installed <br /> to visually verify that the retort is empty.
