All hazardous waste containers shall be marked with the following information: <br />1. the words "Hazardous Waste" <br />2. name and address of generator <br />3. hazardous properties <br />4. physical state <br />5. composition (contents) <br />6. accumulation start date <br />Immediately label these containers and ensure that all containers are marked with all the required <br />information. Since it cannot be determined how long the used antifreeze has been on site, immediately <br />contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of <br />the manifest to the EHD within 30 days of receiving this report. Repeat violation. <br />23. A blue metal container storing crushed used oil filter lack accumulation start date. Used oil filters <br />must be stored in a closed, rainproof container, labeled as "drained used oil filters," and marked with an <br />accumulation start date. Immediately provide accumulation start date on the label and ensure that all <br />used oil filters are managed in this way. Provide proof of corrective action to the EHD within 30 days of <br />receiving this report. Repeat violation. <br />32. Facility failed to have a tank assessment performed for the used oil storage tank installed in 1999. <br />Pursuant to California Code of Regulation section 66265.191 a facility must obtain and keep on file a <br />written tank assessment reviewed and certified by an independent, qualified, and California certified, <br />professional engineer. Submit proof of corrective to the EHD within 30 days of receiving this report. <br />45. Facility lack current phone number for the Office of Emergency Services (OES) in their contingency <br />plan. Also lack listing and description of emergency equipment. Immediately prepare a current <br />contingency plan and submit proof of corrective action to this office within 30 days of receiving this <br />report. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />M .
