C'� � !{(( c � �,/7{!�I <br />f , <br />IF, <br />JUL 1 I Z006 <br />VI ENT HEALTH Date: 7/11/06 <br />PERMITISERVICES <br />Hazardous Waste Inspection Report: Corrections/Compliance statement <br />The following information address all areas noted to be in violation from the inspection <br />performed on 6/9/2006. <br />• #12: The area under the used oil filter crusher has been cleaned and continues to be <br />monitored for cleanliness. Overall housekeeping is being strictly enforced.(see photo <br />#3) <br />• #20,91: Empty Zep drums are now being labeled "Empty" these labels also display the <br />date the drum was emptied, (see photo #1). <br />The used oil storage tank is now labeled with the words "Used Oil" (see photo #2). <br />• #5: All disposals of empty drums through rinsing, crushing and disposal into scrap <br />steel bin by Swift Transportation has stopped, we have contacted our Haz. Mat. <br />disposal company (Safety Kleen) to provide proper removal and disposal of these <br />empty drums. <br />• #45, 47: The contingency plan has been updated with current information including <br />the name of the Emergency Coordinator. (See attached copy) <br />• #28,29: Contact was made with Safety Kleen and they assisted us in obtaining the <br />TSDF in question. Please see attached copy. <br />A safety/trai ng eting was held with all shop employees shortly after the inspection in <br />which all t ab areas wher iscussed.
