Roth Road, Lathrop <br />San Joaquin County Public Health Services Environmental Health Division (EHD) has <br />performed a preliminary review of your submittal for application to remove two <br />underground storage tanks at the above referenced site. <br />Please submit the following documentation prior to issuance of a permit: <br />1) On page 3 of the permit application please specify what class the contractor's <br />license is. <br />2) It states on page 3 of the permit application that Geofon Inc. will be the <br />sampling firm. The sampling firm must be an independent third party from <br />the contractor.
