9.0 CONFINED SPACE ENTRY PROCEDURES <br /> PRIOR TO ENTRY INTO ANY PERMIT-REQUIRED CONFINED SPACE THAT IS <br /> NOT EXEMPTED AND REQUIRES A WRITTEN PERMIT, YOU MUST NOTIFY THE <br /> SENIOR SAFETY OFFICER OR YOUR SUPERVISOR. <br /> Areas that might be considered confined spaces at this site include: <br /> 1. Open Excavations <br /> 2. Turbine, Fill, or Monitoring Sumps. <br /> Confined Space entry is a potential hazardous situation. Extreme care must be taken <br /> when entering or working around confined spaces. <br /> Employees shall be notified of all permit-required confined spaces (PRCS). <br /> Entry in confined space will be conducted in accordance with the following flowchart, <br /> the companies Confined Space entry program and all other applicable Cal/OSHA or <br /> local regulations. <br /> CONFINED SPACE ENTRY PERMIT <br /> GENERAL <br /> Site Location <br /> Identification <br /> of Space <br /> Purpose of <br /> Entry <br /> Permit <br /> Number <br /> HAZARDS <br /> Chemical: possible high concentration of volatile organic vapors <br /> Physical: explosion, heat stress, oxygen deficiency <br /> PRE-ENTRY PREPARATION AND PROCEDURES <br /> Testing procedures/protocols: Measure oxygen, LEL, toxics from top down prior to <br /> entry. <br /> Inspection criteria: Visually inspect product lines for leakage and electrical conduit <br /> for breaks. <br /> Drain/purge requirements: Drain product lines and turbine. <br /> Lockout/tagout: ❑Electrical ❑Mechanical ❑Fluids ❑Energy ❑Other <br /> Disconnect (specify system components): <br /> Internal cleaning: <br /> ENTRY CONDITIONS <br /> Oxygen levels (permitted range): 19.5%<oxygen<23.5% <br /> Flammable atmosphere: <_10% LEL <br /> Toxic atmosphere, specify chemical and concentration: gasoline 50 ppm (PEL) <br /> Test equipment: Multi-RAE three gas sensor 02/LEL/VOC <br />
