Box 281 <br /> Folsom, CA 95763 <br /> Re: Country Club 76, 2575 Country Club Blvd, Stockton, CA SR0061367 <br /> The San Joaquin County Environmental Health Department(EHD) has completed the review of <br /> the application for a permit to replace the tank top sump penetration fittings on the underground <br /> storage tank (UST) system at the above noted address. The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Garrett Backus at(209)468-2986 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Primary piping soap test: One hour pneumatic pressure test at 110% of operating <br /> pressure, or 40 psi, whichever is greater. <br /> b. Secondary piping vacuum test or equivalent. Test the product and vapor recovery <br /> piping. One hour vacuum test per manufacturer's guidelines. If there are no <br /> manufacturer's guidelines, test shall be performed using an applicable method <br /> specified in an industry code or engineering standard. Inspection must be done <br /> before backfill of the excavation. <br /> c. Visual inspection of tank top sumps after interstitial fluid refill or equivalent test. <br /> Inspection must be done before backfill of the excavation. <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br />
