The permit <br />has been approved with the following condiions: <br />1_ Contact the local fire department and the San Joaquin Valley Aa Pollution Control <br />Districtfor any applicable pemits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work- A6 new addervfa supelsade ttfi previous ones. <br />3. Contact Stacy Rivera at (209) 468.3440 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Plsase be <br />aware that inspections for emergency work most be scheduled wftiin five days of permit <br />appreval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. <br />b. Spill bucket test Verify m3ntimml 5 -gallon capacity, and pass a one hour hydrosfsfic <br />test (err equivalent). Proper drainage must also be verified. <br />4, qualified service tecmicians must possess orvwrk under the direct and personal <br />supervision of an Individual physlcefy present at the work site who possesses all of the <br />folming: <br />a One of several permissible valid, current oontrar 's licenses issued by the <br />Contradors State License Board (may be issued to the installation company), or a <br />Tank Tester's ficense issued to the individual by the State Water Board, and <br />b, A valid, cumant California l)cuT Service Technician certificate issued by the <br />International Code Council (ICC), and <br />
