The diesel S1 fuel oil return line, S2 fuel oil supply line, and S3 Vent line smart sensor failed when tested with liquid. The interstitial space of the UST system shall be maintained, as designed, under constant vacuum such that a breach in the primary or secondary containment is detected before the hazardous substance is released into the environment. Correct immediately by having a properly licensed, trained, and certified contractor replace the failed component with a functional component(LG 113-x listed, if applicable)and obtain a permit within one business day from the EHD. If the failed component can't be replaced immediately, there is a possibility that the diesel UST system may be red tagged to prevent fuel inputs. This is a repeat violation, Class ll. 320 HSC 25290.1(c3), 25290.2(c3)Secondary containment not constructed to prevent water intrusion. Water was found in the diesel remote fill sump and diesel fill sump. Randy Roerich confirmed that the boot for the remote fill bucket was not properly clamped on. Secondary containment shall be constructed to prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. During the inspection,Afford-a-test technicians removed liquid and placed it into the facility hazardous waste drum, repositioned the remote fill bucket boot properly, and tightened the boot band clamps to keep the remote fill bucket boot on. Continue to remove any liquid that may enter the sumps, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage accordingly. Corrected on site. This is a repeat violation, Class ll. Overall Inspection Comments: On site to conduct a routine inspection and witness the annual monitoring system certification testing. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 11-8-15. Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written Agreement, if applicable, can be stored at the facility or uploaded into CERS. Note: Technician Zane Nimmo verbally provided his Veeder Root Technician TLS-XXX expiration date as 9-28-17. The Veeder Root list of certified technicians lists his expiration date as 9-26-15. A voicemail was left with Veeder Root requesting confirmation of this technicians actual certification expiration date. If it is confirmed that the technician was not currently certified during the 10-8-15 monitoring system certification testing, then the portion of the testing that requires this certification cannot be accepted.
