The <br /> following items need to be addressed/corrected: <br /> Tank# TA0515887 (20,000 gallon diesel)UST Monitoring Plan: <br /> Tank Description Tank ID# should be TA0515887 not tao515887. <br /> Continuous Monitoring of Piping Secondary Containment should be marked yes. Marking yes should open up <br /> more applicable sections which may require additional information. <br /> Periodic System Testing Secondary Containment testing should be marked no. <br /> Tank# TA0133103 (5,000 gallon diesel)UST Tank Information/Monitoring Plan: <br /> Tank Description Tank ID# should be TA0133103 not tao133103. <br /> Tank Construction Primary Containment should be steel, Secondary Containment should be jacketed. <br /> Vent, Vapor Recovery(VR) and Riser/Fill Pipe Piping Construction Primary Containment should be steel. Riser <br /> Pipe Primary Containment should be steel. <br /> Tank# TA0133103 (5,000 gallon diesel) Monitoring Plan: <br /> Pipe Monitoring is Performed Using the Following Method(s) Pipeline Integrity Testing should be marked yes. <br /> UST Monitoring Site Plan Supplemental Documentation UST Monitoring Site Plans must include a Site Plan <br /> showing the general tank and piping layouts and the locations where monitoring is performed (i.e., location of <br /> each sensor, line leak detector, monitoring system control panel, etc.). Upload needed for each UST. <br /> UST Certification of Financial Responsibility Documentation needs to be updated and re-uploaded. The <br /> insurance listed indicated an end date of 10-31-14. <br /> UST Letter from Chief Financial Officer Supplemental Documentation should list 10,000 per occurrence and <br /> 10,000 annual aggregate coverage. This letter is in support of the use of the UST Cleanup Fund. Therefore, the <br /> Certification of Financial Responsibility should also list the UST Cleanup Fund mechanism.
