PUBLIC HEALTH SERVICES <br /> SAN JOAQUIN COUNTY c° <br /> ENVIRONMENTAL HEALTH DIVISION <br /> Karen Furst, M.D., M.P.H., Health Officer ; P <br /> 304 East Weber Avenue,Third Floor • Stockton, CA 95202 <br /> 209/468-3420 <br /> TO: CONTRACTORS AND OWNERS PROPOSING TO INTERIOR LINE, REPAIR, <br /> OR UPGRADE UNDERGROUND STORAGE TANKS (UST's). <br /> San Joaquin County Public Health Services-Environmental Health Division (PHS-EHD) has established the <br /> following guidelines to assist you in the completion of your tank lining, repair, or upgrade project. <br /> 1. SUBMITTAL OF PLANS <br /> a. Submit 3 completed copies of the permit application along with a "PHS-EHD Service <br /> Request" form to the above letterhead address. <br /> b. A permit will be issued by PHS-EHD upon review and approval of the application and plans. <br /> The required fees must be submitted with the application package. Information in addition to <br /> that presented in the application may be needed to obtain final approval. No work is to begin <br /> on the proposed project until a permit has been issued. The required inspections cannot be <br /> scheduled until a permit is issued. <br /> c. Prior to tank lining, a site assessment must be performed under the direction of PHS-EHD, <br /> Site Mitigation Unit. Please contact the Site Mitigation Unit at (209) 468-3450 for further <br /> information regarding the assessment. <br /> d. To comply with the 1998 upgrade requirements in conjunction with the tank lining, then <br /> equipment and material specification sheets and manufacturer's brochures must be <br /> submitted for all proposed equipment. All proposed leak detection methods and equipment <br /> must meet the requirements specified in 40 CFR 280.40 and have been reviewed by the <br /> State Water Resources Control Board. If the UST facility does not meet the upgrade <br /> requirements by December 22, 1998, then the tank(s) must be closed according to Article 7, <br /> California Code of Regulation (CCR). <br /> e. After permit approval, it is the responsibility of the applicant to notify PHS-EHD at least 48 <br /> hours in advance to schedule the required inspections. <br /> f. Permit from local fire authority must be provided prior to any work. <br /> g. A Certified industrial hygienist must be on site during any tank entries and provide safety <br /> management for the site. <br /> h. Tank lining cannot void the UL label for the tank. UFC Section 5202.3 <br /> i. Tank liner must be able to provide new UL label for each tank upon completion of lining <br /> process. UFC Section 5202.2.3 <br /> j. A complete site safety plan must be provided for work at the site. The PHS-EHD does not <br /> take responsibility for review of this plan and is the responsibility of the contractor for the <br /> completeness and meeting all Federal, State and Local code requirements. <br /> k. The special inspector must personally make all required inspections and measurements. <br /> All inspections by the special inspector must be witness by the PHS-EHD and final <br /> approval for the tank condition and suitability for the lining of the tank will be with the PHS- <br /> EHD. <br /> A Division of San Joaquin County Health Care Services <br />
