The report listed April 29, 2011 as the last test date. The correct <br /> last test date is March 28, 2014 During the monthly inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and document the dates they were done. Ensure f <br /> `that designated operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> 1 i <br /> 1I The designated operator failed to document all the alarms from the attached alarm history on the January 2014 <br /> i !designated operator monthly inspection report and failed to check that they were responded to appropriately. During the <br /> I `monthly inspection, the designated operator shall review the alarm history for the previous month, check that each <br /> alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation <br /> 'taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. This is a minor violation. If water could enter into the <br /> secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The service <br /> technician removed 1 cup of liquid from the middle 87 sump. Ensure that all sumps and annular spaces are <br /> I i maintained free of liquid. This is a repeat violation, Class 11. I <br /> I i I <br /> { Overall Inspection Comments: � <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting I <br /> paperwork, by February 29, 2015 . <br /> Consent to perform the inspection, take photos and collect samples was given by Lawrence Wight the owner. <br /> This facility's designated operator is James Williams (ICC expiration date: 6-4-15 ) .
