The report listed April 29, 2011 as the last test date. The correct last test date is March 28, 2014 During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure f `that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 1 i 1I The designated operator failed to document all the alarms from the attached alarm history on the January 2014 i !designated operator monthly inspection report and failed to check that they were responded to appropriately. The 1 I missing alarms include: Q 2:87 PLLD Shutdown alarm on January 20, 2014, and January 21, 2014. . During the I `monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation 'taken in response to any alarms to the monthly report. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The service technician removed 1 cup of liquid from the middle 87 sump. Ensure that all sumps and annular spaces are I i maintained free of liquid. This is a repeat violation, Class 11. I I i I { Overall Inspection Comments: � Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting I paperwork, by February 29, 2015 . Consent to perform the inspection, take photos and collect samples was given by Lawrence Wight the owner. This facility's designated operator is James Williams (ICC expiration date: 6-4-15 ) .
