Please be advised that the following issues <br /> have been addressed: <br /> #104. Financial responsibility documents have not been submitted to the EHD.The last <br /> financial responsibility document submitted to EHD was for coverage period between <br /> January 1,2012 through January 1,2013. Current financial responsibility documents are <br /> requested to be submitted annually. As noted on your inspection report, copies of the current <br /> financial responsibility documents were provided to you during the inspection. Copies of the current <br /> financial responsibility documents were also sent to your department via U.S.P.S. certified mail, <br /> tracking#71189042955113378774, which was signed for on January 7, 2013, at 12:19 pm. <br /> Therefore, because copies of the current financial responsibility documents were both on-site at the <br /> time of your January 23, 2013, inspection and timely submitted to EHD, we are respectfully <br /> requesting that Violation #104 be rescinded. <br /> #310.About 1-2 gallons of liquid in the premium-octane STP sump and about 1/4 gallon of <br /> liquid in the 87-octane slave STP sump were observed. If water could enter into the <br /> secondary containment by precipitation or infiltration,it must be removed and disposed of <br /> properly. Immediately remove this liquid, make a hazardous waste determination per Title 22 <br /> hazardous waste regulations,and manage it accordingly. As noted on your inspection report, <br /> our BP testing vendor Fastech removed and placed the liquid in the hazardous waste container on <br /> site. Violation #310 was corrected. <br /> 1 <br />
