Take all necessary precautions to ensure testing <br /> is performed in a timely manner. <br /> This is a Class II violation. <br /> 110 CCR 2637(e) Failed to submit secondary containment test report within 30 days. <br /> Secondary containment testing was performed on 1/18/17. A test report has not been submitted to the EHD or the <br /> California Environmental Reporting System (CERS). A copy of the test report must be submitted within 30 days of <br /> the test. Test results were provided while on site. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 111 CCR 2638(a) Failed to test monitoring and leak detection equipment annually. <br /> Annual monitoring system certification and leak detector testing were last performed on 2/25/13 and was completed <br /> on 6/16/17, 52 months past due. These tests are required once every 12 months. Take all necessary precautions <br /> to ensure testing is performed in a timely manner. <br /> This is a Class II violation. <br /> 112 HSC 25284.2 Failed to test spill containers annually. <br /> Annual spill container testing was last performed on 2/25/13 and was completed on 6/16/17, 52 months past due. <br /> This test is required once every 12 months. Take all necessary precautions to ensure testing is performed in a <br /> timely manner. <br /> This is a Class II violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. According to Mr. Bhatia, the <br /> fuel filters were replaced approximately one month ago, but no maintenance records were left on site. Test results <br /> for secondary containment testing conducted on 1/18/17 were not found on site-these results were provided to the <br /> site by the technician at the time of inspection. These records shall be maintained on site for at least three years. <br /> Monitoring records include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br /> maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br /> field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; <br /> (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring <br /> records for the last three years on site and submit copies to the EHD. <br /> This is a Class II violation. <br /> Page 4 of 6 <br />
