This is a Class I violation. 104 CCR 2711 UST Operating Permit Application for Facility and Tank information not submitted or current. UST Facility Operating Permit is not current in CERS. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a repeat violation, Class 11. 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. The response plan is not current and/or not approved by the EHD. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a repeat violation, Class 11. 109 CCR 2637 Failed to test secondary containment. Secondary containment testing was last performed on May 9, 2012 and was due by May 2015 . Testing has not been completed and is one month past due. Secondary containment testing is required once every 36 months. Immediately schedule this test and provide 48 hours notification to the EHD. This is a Class II violation. 111 CCR 2638(a) Failed to test monitoring and leak detection equipment annually. Annual monitoring system certification and leak detector testing were last performed on February 25, 2013 and was due by February 2014 and February 2015. Testing has not been completed and is 28 months past due. These tests are required once every 12 months. Immediately schedule these tests and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be charged at our current hourly rate. This is a Class I violation.
