The annual operating permit fees have not been paid as of today and are due. <br />104. Financial responsibility documents have not been submitted to the San Joaquin County <br />Environmental Health Department (EHD). Current financial responsibility documents are required to be <br />submitted annually and be dated within 2012. The inspector provided the forms. Complete and submit <br />a copy of the financial responsibility by April 15, 2012. <br />106. The monitoring and response plans on site are not current. The monitoring and response plans <br />must be current and approved by the EHD. Secondary containment testing was last performed on 12-23-2008 and was due by 12-2011. <br />Testing has not been completed and is three months past due. Secondary containment testing is <br />required once every 36 months. Immediately schedule this test and provide 48 hours notification to the <br />EHD. <br />109. The annual monitoring system certification was last performed on 1-21-11 and was due by 1-2012. <br />Testing has not been completed and is two months past due. These tests are required once every 12 <br />months. Immediately schedule these tests and provide 48 hours notification to the EHD. Please be <br />aware that a follow up inspection will be charged at our current hourly rate. <br />109. Leak detector testing, under dispenser float and chain testing and spill container testing were <br />performed on 12-21-10 by Affordatest and a test report has not been submitted to the EHD. A copy of <br />the test report must be submitted within 30 days of the tests.
