Financial responsibility documents have not been submitted to the Environmental Health Department <br />(EHD). Current financial responsibility documents are required to be submitted annually. Complete and <br />submit a copy of the financial responsibility to the EHD immediately. <br />6. Approved copies of the monitoring and response plans were not found on site. Copies of these plans <br />shall be retained on site at all times. Immediately locate and retain a copy on site. <br />7. Monitoring system certification, secondary containment test results, and alarm logs for the past three <br />years were not found on site. These records shall be kept on site for at least 3 years. Immediately <br />locate and maintain these records on site and submit copies to the EHD. <br />11. The tank annular sensor, 91 product sump sensor and 87 product sump sensor were found to be in <br />alarm since 12-15-10 and actions have not been taken to address the alarms. The sensors were in <br />alarm before and after the monitoring system testing done today. The sensors do not go out of alarm. <br />All monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br />opportunity. Immediately contact a properly licensed, trained, and certified contractor to address the <br />alarms under permit and inspection of the EHD. Apply for a permit from the EHD for this work within one <br />business day. <br />13. The liquid sensor in the 87 product sump failed to stop the flow of product at the dispenser when <br />tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the <br />flow of product at the dispenser when it detects a leak. Correct immediately by having a properly <br />licensed, trained, and certified contractor replace the failed component with a functional component (LG <br />113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the failed <br />component can't be replaced immediately, there is a possibility that the 87 product UST system may be <br />red tagged to prevent fuel inputs. <br />23c. The November 2009 through November 2010 designated operator monthly inspection reports were <br />not found on site. Designated operator monthly inspection reports for the previous twelve months shall <br />be retained on site. Locate and ensure that copies of the previous twelve months of designated operator <br />monthly inspection reports are maintained on site. Submit copies to the EHD immediately. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'$ CURRENT HOURLY RATE. <br />�EHD dor: e <br />15a <br />Rev 08/10/10 CONTINUATION FORM <br />
