According to the scope of work, 3 UST's will be removed. To date, I have not <br /> received an application for permit to close UST. Please submit application for permit <br /> to close UST, service request form, fee and health and safety plan. <br /> 2. Specify the tank model to be installed. Cut-sheet shows both dry and wet annular <br /> tanks. <br /> 3. The cross-sectional diagram of tank that is site-specific and shows double walls, <br /> location of striker plates and interstitial sensor. Your plan shows a single-walled <br /> tank, and the interstitial sensor is in the tank sump. <br /> 4. Tank manufacturer's installation instruction manual. <br /> 5. Cut-sheet for monitoring system, including all sensors and leak detectors and their <br /> specification. <br /> 6. The location of each sensor; see page 6, item 14 B(a) of application. <br /> 7. Monitoring and response plan as described in CCR 2632(d)(1) &(2). <br /> 8. A site-specific buoyancy calculation. The depth to ground water is approximately 20 <br /> ft. <br /> 9. Site-specific information and cut-sheet for spill container; see page 7 (A) of <br /> application. Application shows existing, plan shows installation, and the cut-sheet <br /> shows CNI and OPW without drainage. <br /> 10. Site-specific information and cut-sheet for overfill prevention. Plan shows Emco <br /> Wheaton, the application shows OPW. <br /> 11. Site-specific information and cut-sheet for piping. Page 8, item 18 of application <br /> indicates existing single-walled fiberglass piping. This is not acceptable. The plan <br /> shows installation of double-walled fiberglass piping. Please specify and submit new <br /> piping information. <br /> 12. Site-specific information and cut-sheet for leak detectors.
