REHS <br />PROGRAM COORDINATORS <br />Margaret Lagorio, REHS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Kasey Foley, REHS <br />San Joaquin County Environmental Health Department (EHD) performed a routine underground storage tank <br />inspection on March 2, 2009 at the above referenced site. <br />An inspection report was issued identifying information to be submitted to bring this site into compliance. This <br />information was required to be submitted by April 2, 2009. This information has not been received resulting in a <br />non-compliant status for this facility. <br />Failure to correct the violations will result in continuing/additional violations. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />current hourly rate ($115). Failure to submit the completed information immediately may result in a re- <br />inspection or further legal action. <br />The missing information includes: <br />r Current UST Facility form (former Form A) <br />r Recordable Release Statement <br />F Current Monitoring and Response plans <br />r Certification of Financial Responsibility and Letter <br />from Chief Financial Officer <br />F Current UST Tank form (former Form B) <br />r Annual monitoring certification, leak <br />detector and spill bucket test results <br />r Maintenance and monitoring logs <br />Permit submitted to repair or replace failed <br />equipment <br />F Designated Operator employee training records r Statement of Designated Operator <br />r' Certification of Return to Compliance (Signed and we Statement of how each violation was or will <br />dated) be corrected <br />The CFO letter submitted should be $10,000 per occurrence and $10,000 annual aggregate coverage. <br />Most forms may be accessed from our web site address, http://www.sjgov.org/ehd/unitiii.htmi.
