Contractor's information. <br />2. Board of Equalization number. <br />3. Question 1-11 on page 5 and question 16a -c on page 7 of the application. <br />4. Plan is not site-specific. Page F2 and F3 of the plan show several tank and <br />piping layouts. Provide site-specific plan, piping layout, cross-sectional <br />diagram of the tank and list of equipment. <br />5. Application indicates 3 tanks to be installed; the plan indicates 5 tanks, a l OK, <br />two 12K, a 15K and a 1 K used oil tank, to be installed. Please provide an <br />accurate number of tanks to be installed. Submit additional form B as needed. <br />6. F 1 of the plan indicates the piping are A -O Smith and Evironflex. Please <br />specify and submit cut -sheet as needed. <br />7. Pressure line leak detector FE Petro #VR 848480-001 and interstitial sensor <br />794390-409 as indicated on F1 of the plan are not on LG 113-12. Please <br />specify and submit cut -sheet for the line leak detector and sensor to be <br />installed. <br />8. The plan indicates Teleflex flex connector, the cut -sheet shows Hosemaster <br />flex connector. Please specify and submit cut -sheet as needed. <br />9. Overfill containment information and cut -sheet. <br />10. Cut -sheet and third party approval for the proposed flex boots. <br />11. F3 of the plan does not show striker plate under all tank openings. Please <br />verify. <br />12. Dimension and cut -sheet for the dispenser pan. The plan and cut -sheet show <br />different dispenser pans. <br />13. The buoyancy calculation is not site specific. The tank size for the calculation <br />on the plan is 12K, in the submittal is l OK. Both calculations do not have <br />engineer stamp and signature. Please submit a site-specific buoyancy <br />calculation with engineer stamp and signature. <br />Please submit above information to EHD.
