The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API,ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 3. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 4. Contact Garrett Backus(209468-2986)to arrange the following inspection(s)(a 48-hour <br /> notice is required): <br /> a. Functionality test on the leak detectors(3 gallons per hour at 10 psi). <br /> b. Functionality test of the dispenser#10 middle Bravo float and chain. <br /> c. One hour hydrostatic test(or equivalent)on the 91 product spill container. <br /> d. If the spill bucket is replaced,verify corrosion protection on the fill riser and spill <br /> container before back filling. <br /> e. If the spill bucket is replaced,verify the five gallon capacity and drain valve <br /> operation of the spill container. <br /> f. If the spill bucket is replaced,a tank secondary containment test is required <br /> following the manufacturer instructions. <br /> 5. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site,at all times while work is being performed, <br /> who possesses all of the following: <br /> a. One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company),and <br /> Page 1 of 2 <br />
