The west side (87 product) Bravo float and chain in the #4 under dispenser containment sump failed <br />to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained <br />to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a <br />leak. Correct immediately by having a properly licensed, trained, and certified contractor replace the <br />failed component with a functional component (LG 113-x listed, if applicable) and obtain a permit within <br />one business day from the EHD. If the failed component can't be replaced immediately, there is a <br />possibility that the 87 UST system may be red tagged to prevent fuel inputs. The owner is tagging out <br />the #4 dispenser to stop dispensing of 87 and 89 octane fuel. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />HIS F LITY IS SUBJECT TO REINSPECTIOJ AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EH spe r: <br />Reiv By: <br />Title: <br />EH 02-003 Rev 08!10!10 CONTINUATION FORM <br />
