Any <br /> changes shall be reported to the EHD within 30 days. The notification identifying all of the designated operators for <br /> this facility was uploaded into the California Environmental Reporting System (CERS). <br /> This was corrected on site. <br /> This is a minor violation. <br /> 205 CCR 2712(i) Failure to have an approved UST response plan available onsite. <br /> The Response Plan was not found on site during the inspection. The Response Plan was downloaded from California <br /> Environmental Reporting System (CERS) and a copy was placed in the file at the facility. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 211 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Tiffany Fite and Tia Fite did not include the "Hire Date". The <br /> designated operator shall train facility employees for which he or she is responsible in the proper operation and <br /> maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> The "Hire Date"was added for the two employees during the inspection. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 309 CCR 2636(f)(1) Monitoring in sump/ UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The diesel Bravo box float and chain in the 9/10 under dispenser containment(UDC) failed to stop the flow of product <br /> at the dispenser when tested. The diesel Bravo box chain was adjusted, retested, and passed.
