The Letter of Chief Financial Officer has not <br /> been submitted. Current financial responsibility documents are required to be submitted annually. Immediately log <br /> into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required <br /> documents. <br /> This is a repeat violation, Class Il. <br /> 106 CCR 2632 Monitoring and response plans not current or approved by the EHD. <br /> The response plan in CERS is missing the second page. The response plan must be uploaded to the California <br /> Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for <br /> review by the EHD. Ensure a copy is available for inspection at any time. <br /> This is a minor violation. <br /> 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br /> The 91 Bravo box float and chain in the 1-2 UDC sump failed to stop the flow of product at the dispenser when tested. <br /> The 87 Bravo float in dispenser 9-10 had to be replaced. All monitoring equipment shall be maintained to activate an <br /> audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br /> adjusted the chain(s) and verified functionality. This was corrected on site. <br /> This is a repeat violation, Class Il. <br /> 319 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the south side of the 91 STP sump and in the north side of the diesel STP sump . If water <br /> could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of <br /> properly. The service technician removed two cups from the 91 STP sump and three cups from the diesel sump. <br /> Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by January 9, 2016 . <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
