ISD will not be installed. The permit has been approved with the following conditions: <br /> Conditions of approval: <br /> 1. Contact the local fire department,local planning or building department and the San <br /> Joaquin Valley Air Pollution Control District for any applicable permits. <br /> 2. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD)for approval prior to commencing work.All new addenda supercede <br /> the previous ones. <br /> 3. A copy of the alarm history prior to commencement of work and after completion of <br /> work must be available for review at the time of EHD inspection. <br /> 4. Contact Garrett Backus(209-468-2986)to arrange the following inspection(s)(a 48-hour <br /> notice is required): <br /> a. Inspection of the monitoring system to verify that the monitoring system alarm <br /> history is still intact after completion of the EVR upgrade. If necessary,a <br /> functionality test must be performed to verify monitoring system meets the <br /> requirements of California Code of Regulations Title 23 and the Health& Safety <br /> Code. <br /> 5. A"Certification of Installation/Modification"form shall be submitted by the owner or <br /> the owners'agent [pursuant to(CCR)2635(d)] before this upgrade will be considered <br /> complete by EHD. <br /> 6. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API, ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 7.
